Trauma Rehabilitation Case Manager – Role Overview
We are seeking a dedicated Case Manager, either part-time or full-time, to provide outpatient support for clients recovering from work-related or auto-related traumatic injuries.
In this position, you will guide injured individuals and their families through every stage of recovery—helping them access appropriate care, coordinate treatment plans, and connect with the right healthcare professionals. You will tailor services to each client’s specific needs while balancing cost-effective solutions and ensuring smooth implementation of care plans. Strong organizational skills, strategic planning, and leadership are essential as you set meaningful goals, monitor progress, and address areas such as family support, vocational guidance, and community reintegration. All care is directed by physicians and prescribed treatment plans.
Key responsibilities include conducting assessments, monitoring mental and physical status, consulting with professionals, supporting life care planning, and providing expert testimony as needed. Accurate documentation of client interactions and professional communications is essential, along with proficiency in electronic medical records. You will manage your own caseload and participate in regular team meetings to ensure continuity of care.
This role includes dedicated office space, a computer, business phone, and email. Benefits include a 401(k) plan and mileage reimbursement. Compensation is competitive and reflects experience.
Required Qualifications
- Registered Nurse (RN) or Licensed Master Social Worker (LMSW) with active state licensure
- Minimum of 1 year of case management experience
- Strong interpersonal and clinical social work skills
- Proficiency with electronic medical records software
- Reliable transportation for field appointments and meetings
Preferred Qualifications
- Experience with traumatic brain injuries (TBI), spinal cord injuries (SCI), or similar populations
- Certified Case Manager (CCM) or Certified Advanced Social Work Case Manager (C-ASWCM)
- 3+ years of medical case management experience
